New Firearms License Appointment Request Form
Please Read Carefully to All the Instructions Below
Applications will only be accepted by appointment, and in person. Please do not mail in application. Bring the application completed signed, and all other necessary documents with you on the day of the appointment.
First Time Applicant for a Firearms License Requirements:
1. Application for Firearms License Completed and Signed. (Application for Firearms License DOWNLOAD HERE)
2. Valid Driver’s license, or Valid Mass State Identification, or US Passport Proving Identification and Confirming Residential Address.
3. Certificate of Completion of Mass State approved Safety Course. Mass Approved Firearms Safety Course List
4. Letter from Applicant to the Chief of Police indicating why you would like a Firearms License, Typed in Business Format with Name, Address, Phone Number Included in the Letter and Signed. This letter should be at least a paragraph in length. Click Here for Business Format Letter Example
5. Two letters of Recommendation; Typed in Business Format with Recommending Person’s Name, Address, Phone Number, and Signed by Recommending Person. These letters should be at least a paragraph in length. Click Here for Business Format Letter Example
6. Money Order payable to the “City of Everett 484 Broadway Everett, MA. 02149” for $100.00.
7. Interview Information: Massachusetts State Law Ch141 S131 Requires that the Incensing Authority (Chief of Police) or his designee Shall personally interview the applicant. This interview will be based on firearms safety information contained in the Massachusetts State approved safety course. Click Here to Download and View the Interview information.
Appointment Process:
7. Click on Link below to book and appointment. Complete the Appointment request area, and click on ‘Submit Request’.
8. Check your email daily for a email reply from the Everett Police Dept with a list of Dates and Times available for your Appointment. (You may need to Check your SPAM Folder for an reply email).
9. Choose a Date and Time listed in the email. Reply back to the Everett Police with the date and time you choose. The Everett Police will then respond back with a conformation email.
10. On the day of you appointment you will need to bring your Application Fully Completed and Signed, Money Order, Valid Mass ID, and any Letters or Documents required.
Please ensure All documents are Completed, Signed & Dated, before arriving for your appointment. This will ensure a speedy appointment process.
